Request for Credit Form

If you have incorrectly ordered, received, or no longer require any goods supplied by Superior Healthcare, please complete and return the attached form ensuring all fields are completed.

Superior Health Care has a return policy of 7 days from the date of invoice. If your request falls outside these terms you may still complete and return this form to us.  A representative from Superior will then contact you and advise you of the outcome. If an exceeded application is approved it may incur a re-stocking fee which will be advised.


Catalogue Request Form

To receive an updated price list please complete this form and return it to our office. We will then send you the appropriate list you require.

Brochure / Sample Request Form

If you require any brochures or samples on products you currently purchase or are interested in please complete this form and return to our office. We will, to the best of our ability supply these for you.

Site Visitation Request Form

As we are a supplier to a large amount of Aged Care Facilities in Victoria we don't always get to see you as much as we would like. If you would like our company representative to come our and see you for any reason, please complete this form and return it to our office and we will organize a suitable time to come and visit you.

Facility Order Form

This is a blank order form which includes our company contact details. You may use this form or your own form to place orders. We also offer a personalized order form service in which we report on your previous purchase history. We then create an order form with all product details so all you have to do is add in the quantities required. This form can be continuously added to at your request.

Resident Order Form

This is a blank order form which enables facilities to order for individual residents and provide us with direct billing details.

Standing Order Form

This form can be used when a product / or products are required regularly. Simply complete this form once, return it to our office and we will process your order for delivery on your chosen delivery dates.

Please specify your preferred delivery day (ie. Mon-Fri) and the frequency required (ie. Weekly, Fortnightly, Monthly).

A complete listing of our product range. Our product catalog is listed by product category (eg. Bandages & Dressings) then sorted by item code (eg. BDAX100, BDAX425/1).


Nutritional Product Catalogue

This catalogue gives a listing of our full nutritional range. Items are categorized by manufacturer (eg. Nestle) then sorted by product code (eg. FCNO156A/1, FCNO157/1).

Continence Product Catalogue

This catalogue includes all continence products stocked by Superior Health Care. Items are categorized by manufacturer (eg. Tena) then sorted by product code (eg. INCT200/14, INCT200/56).
